Why this matters now

Several Medicare changes are already affecting organizations today.

Effective January 1, 2025, the Centers for Medicare & Medicaid Services (CMS) expanded eligibility for Part D Medication Therapy Management (MTM), increasing the number of beneficiaries eligible for annual Comprehensive Medication Reviews from approximately 3.6 million to roughly 7.1 million.

At the same time, CMS is calculating performance for two Pharmacy Quality Alliance measures that become part of the 2027 Medicare Star Ratings:
• Concurrent Use of Opioids and Benzodiazepines
• Polypharmacy: Use of Multiple Anticholinergic Medications in Older Adults

Performance for those measures is based on 2025 measurement-year data, meaning organizations are already being evaluated.

Separately, the MTM Program Completion Rate for Comprehensive Medication Review moves temporarily to the CMS Display Page for the 2025 and 2026 measurement years before returning as a Star Ratings measure beginning with the 2029 ratings using 2027 performance data.

The challenge isn’t prescription drugs—it’s everything else

CMS defines a Comprehensive Medication Review as an annual review of a beneficiary’s complete medication regimen, including prescription medications, over-the-counter medications, herbal products and dietary supplements.

Prescription medications are typically available through pharmacy claims and electronic health records. Supplements, vitamins and many over-the-counter products are not. As a result, pharmacists frequently rely on patient recall to identify products that may meaningfully affect medication safety.

That becomes increasingly important when evaluating cumulative medication burden. For example, commonly purchased over-the-counter allergy medications may contribute to anticholinergic burden even though they may never appear in a prescription record.

The missing piece is supplements.

What IntractMD does

IntractMD screens prescription medications together with dietary supplements, vitamins and foods in a single query, prioritizing clinically actionable risks while producing a Composite Polypharmacy Risk Score (CPRS) across eight clinical dimensions, including bleeding risk, cardiac risk, central nervous system effects, CYP450 interactions and renal/hepatic considerations.

The platform draws on a precomputed knowledge base containing more than 173,000 drug-pair analyses covering approximately 590 commonly prescribed U.S. medications. Drug terminology is standardized using RxNorm, and interaction content is grounded in FDA-approved prescribing information. Results are available in both English and Spanish and can be shared directly with patients following a medication review.
